Completing the Application for Payment Plan

To be able to go through collections the following items are required:

• Physical Address
• Primary Telephone Number (If you do not have a primary #, you will be required to provide 3 references)
• Two References with Name, Complete Address, and phone number that does not match your number

If you cannot complete these items on the application you will not be allowed to go through collections process.

• Please do not sign or date this application till you turn it in to the collector in the court room on the day of plea.
