Obtain a Death Record
The Brazoria County Clerk’s Office records consist of all the deaths recorded since 1903 to the present. Search Brazoria County’s Death Index. The Brazoria County Clerk’s Office can issue certified death certificates for individuals who passed away in Brazoria County only. You must have a valid ID, payment and be a qualified applicant to obtain the record.
If the County Clerk’s Office does not receive the Death Application and/or Identification within 48 hours after the payment has been submitted, a refund will be processed (convenience fee excluded).
Qualified Applicant
The parent, child, sibling, spouse, grandparent, informant or managing conservator.
A death record is public information and available to the public on and after the 25th anniversary of the date of death as shown on the record.
Proper Identification
The qualified applicant must present a Valid Government or State Issued Picture Identification.
Effective 9-1-15, Senate Bill 200 requires a person applying for a certified copy of a birth or death record by mail, email or fax to provide notarized proof of identity before the record can be issued.
Cost
- $21.00 for the First Certified Copy
- $4.00 for Each Additional Copy

Certificate Request by Mail
To obtain a certificate by mail:
Complete the Birth/Death Application Form or send a letter providing the following information:
- Full Name on Death Record
- Date and Place of Death
- Full Name of Parents (if known)
- Name, Address and Phone Number of Requester
- Relation to Person on the Death Record
Include a copy of the applicant’s valid picture ID, signed and notarized proof of identification form and an accepted method of payment. Please provide a legible return address and day time telephone number (if available).

Certificate Request Paid by Credit Card
To obtain a certificate by Credit Card (MasterCard, Visa, & Discover):

Print and Complete the Birth/Death Application Form.

Process Full Payment - Type of Transaction: Vitals (Certified Payments charges a convenience fee).

Write the Certified Payments confirmation number on the Death Application.

Submit the completed Death Application with a copy of the applicant’s valid picture ID by:

Expedited Mail Service
To request Expedited Services for a Certificate Paid by Credit Card:

Print and Complete the Birth/Death Application Form.
